Home Health Physical Therapist

We are seeking a skilled Physical Therapist to join our team in providing in-home patient care in Danbury, CT and surrounding areas. As a Physical Therapist with LHC Group, you will have the opportunity to build trusted relationships with patients and their families, while delivering high-quality, cost-effective care that supports our patients when and where they need it.
Responsibilities:
- Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions.
- Develop and implement treatment plans to improve and enhance patient well-being.
- Evaluate patient progress and make adjustments to treatment plans as needed.
Requirements:
- Current licensure to practice physical therapy in the state of practice.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
