At PeaceHealth, we are seeking a highly skilled Clinical Care Coordinator to join our team in an exciting opportunity for a RN Care Management role. This position will be responsible for coordinating and facilitating the discharge plan of care for selected patient populations across the continuum of illness, promoting effective utilization and monitoring of healthcare resources.

The ideal candidate will possess a strong background in clinical nursing, with a minimum of 3 years of experience in an acute care hospital setting. A degree from an accredited school of nursing is required, and a Bachelor of Science in Nursing or Master's Degree in Nursing is preferred.

This is a full-time, 0.90 FTE, Night position that requires working effectively within an interdisciplinary team to achieve optimal clinical, financial, and satisfaction outcomes. The successful candidate will have excellent interpersonal skills, ability to communicate effectively, and proficiency in applying technical principles and concepts.

We offer a competitive salary range of $56,439 - $103,856 per year, based on education, training, and experience. In addition to the base salary, you may also be eligible for a hiring bonus and relocation assistance.

This position is represented by a collective bargaining agreement and is subject to the terms of the agreement.
