A Comprehensive Job Description:

We're seeking skilled Medical Care Specialists to join our team as In-Home Assessment Professionals. As a key member of our HouseCalls product, you'll play a vital role in delivering annual clinical assessments to patients in the comfort of their own homes.

This critical preventive care helps identify and reduce health risks for patients, coordinating appropriate follow-up care to improve overall well-being. You'll be part of a dynamic team that's uniting industry-leading solutions to build an integrated care model addressing physical, mental, and social needs.

We offer flexible provider schedules available between 20-40 hours per week, allowing you to achieve a healthy work-life balance. With no on-call requirements, weekends, or holidays, you can enjoy a more predictable schedule.

Responsibilities:
  • Conduct thorough in-home assessments, including medical history evaluation, systems review, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing when necessary.
  • Identify diagnoses to inform care management and active medical management.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment.
  • Communicate assessment findings to inform PCPs about potential gaps in care.
  • Educate members on disease processes, medication, and compliance.

Requirements:
  • Active NP or PA license or ability to obtain by start date, unencumbered and free of any disciplinary actions.
  • Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date.
  • Ability to complete physical job requirements, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • May be requested to obtain additional licensure in other geographic areas.
  • 1+ years of clinical experience in their highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home Health care or home visit experience.
  • Ability to travel up to 5% within licensed states for additional incentives (with notice, based on business needs).

What We Offer:

A competitive salary of $115,000-$130,000 per year, depending on experience and qualifications, plus benefits and opportunities for growth and professional development.
