Job Title: Registered Nurse

VNA Health Group is seeking a skilled Registered Nurse to provide high-quality patient care in the comfort of their own homes.

Job Summary:

The successful candidate will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing ongoing support and education to patients and their families.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans to meet patients' unique needs
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Collaborate with patients' healthcare teams to ensure seamless care coordination
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Current RN licensure in the state of New Jersey
  • 1+ year of experience in home health care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
