Job Summary

We are seeking an experienced Licensed Practical Nurse (LPN) to join our team. The successful candidate will work collaboratively with our healthcare professionals to deliver high-quality, compassionate care to our patients.

Responsibilities

  • Provide personalized bedside care to meet the unique needs of each resident
  • Administer medications safely and accurately, documenting therapeutic effects as required
  • Assist residents with their comfort and well-being, promoting a positive quality of life
  • Prepare and administer injections, enemas, and other medical interventions as needed
  • Collect samples and monitor catheters, ensuring accurate and timely documentation
  • Provide wound care and dressing changes, promoting optimal healing and tissue repair
  • Support staff development through coaching and mentoring, enhancing our team's skills and knowledge
  • Maintain accurate and comprehensive nursing notes, reflecting the care plan and treatment administered
  • Gather resident information and data for use in comprehensive assessments, MDS+, and RAPS
  • Admit, transfer, and discharge residents according to established procedures and protocols
  • Request and arrange diagnostic and therapeutic services as ordered by physicians, following established procedures
  • Administer professional services such as catheterizations, tube feedings, suctioning, and vital signs

Requirements

  • Current LPN license in the state of employment
  • Able to pass background and drug tests
  • Up-to-date first aid and CPR certification
  • Up-to-date physical and TB test results
  • Willingness to work evenings, weekends, and flexible schedules
  • Exceptional staff training and customer service skills
  • Compassionate and friendly demeanor, with a strong focus on patient-centered care
  • Able to critically think and problem-solve in routine and emergency situations
  • Experience supervising direct support staff, with excellent communication and teamwork skills
