Advanced Practice Provider Palliative Care Specialist

**About the Role**
As an Advanced Practice Provider for Palliative Care at OSF HealthCare, you will provide high-quality, compassionate care to patients with serious or life-limiting illnesses and their families. You will work collaboratively with a multidisciplinary team of healthcare professionals to address the physical, emotional, spiritual, and social needs of patients and caregivers. Key responsibilities include:
- Conducting comprehensive assessments to develop individualized care plans
- Providing education and counseling to patients and caregivers
- Participating in quality improvement initiatives, research, and education activities related to palliative care
- Care will be provided at patients' homes

**Qualifications**
- Master's degree or higher in nursing or physician assistant studies from an accredited program
- Current Illinois license as a nurse practitioner or physician assistant
- Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
- Minimum of two years of experience as an APP in primary care or a related field is preferred
- Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
- Proficiency in electronic medical record (EMR) systems
