Support Staff for Recovery Services

Job DescriptionWe are seeking Support Staff to join our team of dedicated professionals who provide substance use, mental health, and primary care treatment to patients in need. As a Support Staff member, you will be responsible for:
- Providing services identified in the IM-CANS (Individualized Medication and Care Service Plan) to meet consumer needs that are consumer-driven and meet program requirements;
- Maintaining a minimum of assigned consumer service levels;
- Completing pre-authorizations and re-authorizations of services in a timely manner;
- Providing services in the location most conducive to the consumer's level of change, including their apartment or another community location;
- Linking consumers to additional clinical resources when needed;
- Completing clinical screenings of new tenant housing referrals when assigned;
You will also promote the recovery model of treatment, including trauma-informed care, hope, respect, empowerment, health/wellness, and spirituality/connectedness by assisting consumers with regaining control over their own lives through self-directed recovery.
Required Skills and QualificationsTo be successful in this role, you must have:
- A high school diploma or equivalent and six months of peer or social service experience, or;
- A high school diploma or equivalent, and five years' social service or peer experience, or;
- A Bachelor's degree in counseling, social work, education, psychology, or a related human service field;
- A valid driver's license, private automobile insurance, and be insurable when applicable;
- Basic computer skills, including MS Word, MS Excel, Outlook, Adobe Acrobat, and an electronic medical record (EMR);
- Satisfactory completion of Chestnut's background checking process.
