Licensed Practical Nurse

Brockton Neighborhood Health Center seeks a highly skilled Licensed Practical Nurse to join our dynamic team in Adult Medicine. As a LPN, you will play a vital role in providing exceptional patient care and promoting health education in our multicultural community.
About the Role:
- Provide professional nursing care to patients of all ages, adhering to prudent nursing standards and acceptable practice guidelines.
- Maintain efficient patient flow, assist providers, and other staff members as needed.
- Perform tasks and procedures within the scope of a Licensed Practical Nurse (L.P.N.) and legal guidelines of the Nurse Practice Act.
Responsibilities:
- Patient Preparation and Vital Signs: Prepare patients for visits, take vital signs, including blood pressure, temperature, pulse, orthostatic blood pressure and pulse, and chart documentation in EHR.
- Measurements and Documentation: Measure height, weight, and other necessary measurements; document reason for visit, medication review, allergy review, and medication administration.
- Patient Procedures: Perform audio acuity testing, ear irrigation, electrocardiogram (EKG), visual acuity testing, injections, and oxygen administration.
- Laboratory Tests and Patient Services: Collect and process specimens, perform CLIA Waived Tests, Pap Smear Handling, and participate in the PCMH, including follow-up of hospital discharges and directing medical assistants as needed.
- Quality Assurance and Improvement: Participate in quality assurance monitoring, goal setting, and devising policies, procedures, and protocols to improve patient care.
Requirements:
- Graduate of an accredited school of nursing
- Current state licensure to practice in Massachusetts
- CPR certified
- Maintains licensure and certifications
Working Conditions:
The Licensed Practical Nurse works in a busy community health center in a downtown business district, sometimes requiring long hours. Physical demands include lifting small equipment and boxes up to 40 pounds, mostly standing and walking, with sufficient visual and hearing acuity for frequent reading and conversation.
Salary Range: $29 - $34.12 per hour
