**Job Summary:**
We are seeking an experienced Registered Nurse (RN) to fill the role of a Healthcare Case Manager. This position is responsible for providing high-quality patient care and ensuring seamless transitions between healthcare settings.
Estimated Salary: $93,000 - $114,000 per year

About this Role:
This Healthcare Case Manager RN will work collaboratively with physicians, interdisciplinary teams, and patients to promote positive outcomes. Key responsibilities include:
- Demonstrating critical thinking skills when utilizing the nursing process, based on research and evidence-based outcomes to meet patient needs.
- Establishing collaborative partnerships with patients to examine patterns of healthcare needs and make informed decisions about treatment options.
- Advocating for patients, educating them about treatment options, community resources, and self-care skills to promote independence.
- Facilitating communication and coordination between healthcare providers to minimize fragmentation in services.
- Working independently with minimal supervision and taking decisive action in fast-paced environments.
Requirements:
To be successful in this role, candidates should have:
- A graduate degree from an accredited or state-approved Registered/Professional Nursing program.
- State licensure as a Registered Nurse (RN).
- At least 2 years of nursing experience, preferably in acute care and case management.
