Licensed Practical Nurse

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Rochester West Health Services. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the well-being of our residents.
Job Summary:Under the supervision of the RN Unit Manager or DON, the LPN will be responsible for providing nursing care to assigned residents/patients, coordinating care with other healthcare professionals, and maintaining a safe and clean environment. The ideal candidate will have excellent communication skills, a strong work ethic, and a passion for delivering exceptional patient care.
Responsibilities:- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
- Develop and implement individualized care plans, in collaboration with the interdisciplinary team.
- Conduct thorough assessments, identify care needs, and prioritize interventions.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Maintain accurate and detailed records, including progress notes and medication administration records.
- Licensed in the State as an LPN.
- Current CPR certification.
- Able to use necessary supplies and equipment, including feeding pumps, oxygen, treatment trays, and medication carts.
- Able to calculate drug dosages and administer medications accurately.
- Meets all health requirements imposed by law.
- Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers, and other center staff.
- Able to read, write, speak, and understand English.
