About the Position

We are seeking a dynamic and dedicated Licensed Practical Nurse - Panel Manager to join our team at the Mid County Health Clinic. This is an exciting opportunity to manage day-to-day operations, inspire a diverse team of professionals, and ensure financial sustainability in alignment with our strategic plan.

Key Responsibilities
  • Direct Client Care: Provide health education on various topics, conduct focused assessments, and assist with procedures.
  • Care Coordination: Schedule appointments, manage referrals, communicate with providers and the care team, and follow up on client needs.
  • Panel Management: Proactively manage client needs using electronic reports to identify needs for preventative screenings, chronic disease monitoring, and immunization forecasting.
  • Team Leadership: Lead and direct staff to ensure smooth clinic flow.
  • Administrative Tasks: Manage communication channels, patient reminders, and meticulous documentation in patient charts.
Requirements
  • Associate's degree or a Bachelor's degree in nursing from an accredited nursing program.
  • LPN licensure.
  • BLS CPR certification.
  • A minimum one (1) year of nursing experience.
  • Swahili / English language, cultural knowledge/experience.
