Responsibilities:- Provide skilled nursing care to patients in their homes, as per physician orders.
- Conduct thorough assessments of patients' needs and develop individualized care plans.
- Implement and update care plans as necessary, ensuring that patients receive the best possible care.
- Collaborate with healthcare teams to coordinate patient care and ensure seamless transitions.
- Educate patients and families on disease management, self-care techniques, and prevention strategies.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.
- $26-$32/HR salary range.
