Job Summary
This Home Health Aide II position plays a vital role in providing necessary care to patients as designated by a written care plan developed by our RN Case Manager. You will perform activities that assist patients in meeting their basic human needs and daily living tasks under the direction of our registered nurse.
Key Responsibilities
- Assist patients with personal care and daily living activities, such as bathing, oral hygiene, toileting, feeding, dressing, exercises, ambulation, transfers, and medication administration.
- Provide normal household services essential to patient care at home, including meal preparation, laundry, and light housekeeping duties.
- Report any recognized changes in the patient's condition to the RN Case Manager or Registered Nurse designee.
- Communicate effectively with patients and families, adhering to confidentiality policies regarding patient scheduling, work assignments, and agency-related information.
Requirements
- A High school diploma or GED is required. Certificate to practice as a Home Health Aide in Connecticut or CNA certification with the Connecticut State Nurse Aide Registry is also necessary.
- Enrollment in or completion of a Basic Approved Training Program as defined by Licensure Regulations or evidence of meeting exemption criteria is required.
- Previous experience as a CNA/HCA is preferred.
- Ability to read, write, and speak English, comprehend written and oral instructions, and drive a car with a valid driver's license and insurance.

Estimated Salary Range: $45,000 - $55,000 per year (dependent on location and experience).
