At Axesspointe Community Health Center, we are seeking a skilled Ambulatory Mental Health Specialist to join our team. This is a full-time position with an estimated salary range of $65,000 - $85,000 per year.

Job Summary:

The Ambulatory Mental Health Specialist will provide direct client care services and community support to individuals with severe mental disabilities. As a generalist, the specialist will assess client needs, develop and implement individual service plans, and provide psychotherapy/counseling services through individual, group, and family methods.

Key Responsibilities:

  • Evaluate client needs for services in regard to appropriateness, urgency, and recommended action
  • Conduct and write diagnostic assessments according to VCS policies and procedures
  • Develop and implement Individual Service Plans based on diagnostic assessment findings
  • Provide psychotherapy/counseling services to clients and significant others
  • Review and update ISPs, progress, and diagnoses on a scheduled basis
  • Enter and maintain daily schedule into computer as required
  • Achieve and maintain the Agency's established productivity standards
  • Provide crisis intervention, pre-hospital screening, and emergency services as needed
  • Make referrals to other services within the community and within the Agency
  • Attend and participate in staff meetings, supervision, and training sessions

  • Strong assessment and treatment planning skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a team
