Job Summary:

We are seeking a dedicated Registered Nurse to join our team at Maxim Healthcare Services. As a Nurse Care Coordinator, you will play a critical role in providing high-quality patient care and support to our pediatric clients in the comfort of their own homes.

About the Role:

This is a full-time opportunity for a licensed Registered Nurse to work with our pediatric patients, providing skilled nursing care and support. You will be responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with our interdisciplinary team to ensure seamless care coordination.

Responsibilities:

- Utilize the nursing process to plan, implement, and evaluate patient care and services provided to clients receiving contracted services.
- Initiate the plan of care when directed and request revisions as patient status and needs change.
- Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals.
- Intervene in response to changes in patient condition and report changes to appropriate physicians or supervisors.
- Administer medications and treatments as prescribed, monitor and document effectiveness of intervention.
- Review plan of care and program protocols and identify areas for healthcare teaching/education, care coordination, goal setting, and discharge planning needs.

Requirements:

- Current Registered Nurse (RN) License for the state in which the nurse practices.
- Current Health Certificate (per facility Registered Nurse (RN) contract or state regulation).
- Current PPD or Chest X-Ray.
- Current BLS card.
- One year prior Registered Nurse (RN) experience preferred.
