Job Description:

In this exciting role as a Certified Community Health Specialist, you will play a vital part in bridging the gap between health services and communities. As a frontline outreach worker, you will work closely with high-risk populations, providing education, counseling, and advocacy to promote positive health outcomes.

Key Responsibilities:

  • Provide integrated outreach to individuals with mental illness, substance use disorders, and marginalized health disparities.
  • Maintain frequent communication using various methods like in-person, calls, texts, and letters.
  • Mentor contacts in using service providers, assist with transportation, harm reduction, empowerment techniques, and liaison services.
  • Connect contacts to required IOS program services, self-help groups, and treatment teams.
  • Coordinate services for contacts monthly, emphasizing person-centered approaches and promoting support network development.
  • Enroll eligible individuals into IOS programs, maintain communication, and coordinate appointments.
  • Refer contacts to appropriate community providers, facilitating connections.
  • Provide transportation adhering to traffic laws and utilize SAMA methods during crisis situations.
  • Complete progress notes for each contact and file them in their chart.

Requirements:

  • A minimum of 2 years of experience working in a community-based setting, preferably with serious mental illness, substance use disorders, and HIV/STI prevention and testing.
  • A valid Texas Driver's License and certification as a Certified Community Health Worker within 6 months of hire date.
  • Possess knowledge of contacts' needs and characteristics, assessing needs and creating comprehensive treatment plans.
