Job Summary:
The Nurse Care Coordinator - Patient Safety Expert plays a critical role in ensuring the delivery of high-quality, patient-centered care at ECHN. This position involves coordinating patient care with the interdisciplinary team, patients, and families to promote positive outcomes and patient safety. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a fast-paced environment.
Key Responsibilities:
- Comprehensive Patient Assessments: Conduct thorough assessments of patient conditions to identify needs and develop individualized care plans.
- Patient Safety Expertise: Provide guidance and support to patients and families to promote safe practices and prevent adverse events.
- Care Coordination: Collaborate with the interdisciplinary team to ensure seamless transitions of care and effective communication.
- Quality Improvement Initiatives: Participate in quality improvement initiatives to enhance patient care and safety.
- Professional Development: Stay up-to-date with the latest evidence-based practices and guidelines to ensure optimal patient care.
Requirements:
- Graduate of an Accredited School of Nursing: Must have a degree in nursing from an accredited institution.
- Current Licensure in Connecticut: Must be licensed to practice as a registered nurse in the state of Connecticut.
- Minimum One Year of Nursing Experience: Must have a minimum of one year of experience in a relevant nursing role.
- Basic Computer Skills: Must have basic computer skills and experience with electronic medical records.
