Job Summary

We are seeking an experienced and compassionate Hospice Nurse Specialist to join our team. In this role, you will provide skilled nursing care and support to patients and their families, ensuring they receive the best possible care during their end-of-life journey.

Responsibilities
  • Provide skilled nursing care to patients in their homes, hospitals, or nursing facilities
  • Assess patients' needs and develop individualized care plans
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Provide emotional support and education to patients and their families
  • Maintain accurate records and reports
Requirements

To be successful as a Hospice Nurse Specialist at Corewell Health, you must have:

  • A High School Diploma or equivalent
  • Completion of a State of Michigan approved CNA Training Course
  • PROMETRIC Certified Nursing Assistant certification within 90 days
  • A valid Driver's License within the State of Michigan
