Job Summary

The Care Team Nurse functions as a liaison between the patient and members of the Care Team in a joint effort to optimize the patients' health status and quality of life. This role requires routine and emergent nursing functions pertinent to efficient, ambulatory patient care, while following the guidelines set forth by the Colorado Nurse Practice Act.

Responsibilities
  1. Participate as part of the interdisciplinary team to provide patients with appropriate services and ensure coordination of these services through the nursing process, to include assessment, planning, facilitation, complex focused care coordination, evaluation, advocacy, and comprehensive care plan development to meet a patient's complex needs.
  2. Function within the interdisciplinary team to support team-based care in assigned clinics, as well as other clinic locations.
  3. Assess high-risk and rising-risk patients consistent with the current Policies & Procedures for Care Management.
  4. Assess and triage per protocol for the appropriate level of care based on comprehensive risk assessment, including diagnosis, clinical judgment, and outcome goals.
  5. Identify and link patients to available resources, social support systems, family support, etc.
  6. Provide education to patients, families, and caregivers.
  7. Encourage engagement and self-management of patients with regards to their own health.
  8. Provide outreach to individuals, patients who are inactive in their healthcare, as well as the community.
  9. Provide in-reach to patients who are presently inactive in their healthcare.
  10. Facilitate access to quality and cost-effective care.
  11. Demonstrate an ability to utilize technology, including Electronic Health Records, state registries, and Health Information Exchange.
  12. Maintain documentation of all interactions accurately and completely, implementing appropriate protocols for patients' charts and billing practices.
  13. Understand the immunization schedule and support clinic staff and patients.
  14. Assist in the support of organizational clinic training needs for all clinical staff as assigned.
  15. Routinely attend trainings and meetings as recommended and required.
  16. Routinely participate in peer review, audits, and chart audits as assigned.
  17. May be assigned to participate in precepting and training staff and students.
Qualifications

A Graduate of an accredited Nursing Program is required; BSN preferred. Current Colorado RN License, Current CPR (BCLS) certification, Valid Colorado driver's license, and proof of insurance are also required. Spanish bilingual preferred.

Compensation

$27.39 - $39.43 hourly, based on experience. A sign-on bonus of $5,000 will be paid in installments over 9 months. A loyalty bonus of $5,000 will be awarded upon completion of one year of service, with additional bonuses paid out every three months thereafter.
