**Overview**

We are seeking a skilled Patient Care Coordinator to join our team at Concord Hospital, Inc. This role is a fantastic opportunity for individuals who are passionate about delivering high-quality patient care and want to make a meaningful difference in the lives of others.

**Estimated Salary:** $45,000 - $60,000 per year (based on location and experience)

**Job Description**

As a Patient Care Coordinator, you will be responsible for providing direct and indirect patient care duties under the guidance and supervision of our registered nurses. Your primary focus will be on ensuring that patients receive the best possible care, from simple to complex tasks, tailored to their specific needs.

**Key Responsibilities**

  • Perform activities of daily living and personal care needs under the direction of our registered nurses.
  • Document care provided and report observations of patient's physical, social, and mental status.
  • Ensure cleanliness, safety, and supplies of material and equipment in patient care areas.
  • Adhere to safe patient handling policy and demonstrate time management skills.

**Requirements**

To be successful in this role, you will need:

  • A High School Diploma or equivalent (GED) preferred.
  • Proof of LNA Education Certificate and Final Report of Written and Clinical Competency Testing Results.
  • Certification required: Current American Heart Association Basic Life Support for Healthcare Providers certification.
  • Licensure required: Current New Hampshire State LNA license.
