Job Summary:
We are seeking a skilled Behavioral Health Nurse - RN to join our team. The successful candidate will be responsible for coordinating care referrals, ensuring seamless transitions for patients, and providing compassionate and effective care to patients in need.
Responsibilities:
- Collaborate closely with our care coordination team to facilitate referrals to other facilities or higher levels of care.
- Play a vital role in arranging final disposition arrangements, ensuring seamless transitions for patients.
- Utilize expertise in behavioral health to provide effective care to patients in need.
- Ability to effectively assess patients holistically for behavioral health needs, work with physicians for plan of care, transfer and discharge planning needs, real and/or potential.
- Monitors for return patients for appropriate outside care management.
Requirements:
- 1 year behavioral health experience.
- Ability to complete psychosocial assessment, exceptional communication and problem-solving skills, general knowledge of utilization management, understanding of levels of care and community resources.
- Demonstrated skills of organization, teamwork, strong interpersonal and written communication skills, and flexible work schedule, and be self-motivated.
- Must be proficient in word processing programs and use of clinical software.
- BSN preferred. Certification for Delaware Commital Process preferred.
