Job Summary

Thrive Skilled Pediatric Care LLC is seeking a dedicated and compassionate Pediatric Nurse to provide high-quality clinical home care to medically fragile children. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide one-to-one care to patients in their homes, promoting their physical, emotional, and developmental well-being.
  • Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams.
  • Administer medications, perform medical procedures, and provide wound care as needed.
  • Monitor patients' conditions, reporting any changes or concerns to healthcare providers.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
Requirements
  • Current nursing license and current CPR certification.
  • Experience working with medically fragile children and their families.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on providing exceptional patient care.
