Senior Home Health Nurse Specialist

Estimated SalaryWe offer an estimated annual salary of $85,000 - $110,000 based on experience and qualifications, along with opportunities for career advancement and professional growth.
About the RoleThis position requires a compassionate and dedicated RN to provide high-quality patient care in the comfort of their own homes. The ideal candidate will possess excellent communication skills, be able to work independently, and demonstrate a commitment to delivering exceptional patient outcomes.
Key Responsibilities- Develop and implement personalized care plans tailored to each patient's unique needs and goals.
- Conduct thorough assessments and evaluations to identify potential health risks and develop strategies to mitigate them.
- Administer medications, perform wound care, and provide other medical interventions as necessary.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless coordination of care.
- A current license as a Registered Nurse in MD.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills and ability to communicate effectively with diverse populations.
