Responsibilities

Gather pertinent data and perform examinations to determine treatment diagnoses
Develop individualized plans of care through coordination with patients and families
Implement and modify plans of care based on reassessment and patient response
Coordinate care and communicate effectively with the interdisciplinary team for all aspects of patient care
Establish appropriate discharge plans

Requirements

Occupational Therapy Licensure in the State of Minnesota or temporary license in the State of Minnesota
Active Minnesota Basic Life Support Certification

Preferred Qualifications

Master's Degree in Occupational Therapy
1 year(s) of OT experience post-graduation
Certification with the National Board for Certification in Occupational Therapy
