- Our HouseCalls product provides annual clinical assessments to patients in the comfort of their homes, helping identify and reduce health risks while coordinating appropriate follow-up care.
Your Role:
As a HouseCalls Statewide Traveler, you will conduct in-home assessments, perform evidence-based practice screenings, and communicate findings to inform members' primary care physicians. Your expertise will be essential in identifying diagnoses, formulating lists of current and past medical conditions, and educating patients on disease processes, medications, and compliance.
Key Responsibilities:
- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform point-of-care testing (as applicable) and evidence-based practice screenings.
- Identify diagnoses to inform care management and active medical management in furtherance of treatment.
- Communicate assessment findings to inform members' primary care physicians of potential gaps in care.
- Educate patients on disease processes, medications, and compliance.
Requirements:
- Avoid unnecessary travel, work-life balance flexible scheduling offered where providers can flex their time over the 7-day work week, no weekends or holidays required.
- An active NP or PA license or ability to obtain by start date, with licensure unencumbered and free of any open/unresolved disciplinary actions.
- Able to complete physical requirements of the job, including lifting a 30-pound bag, navigating stairs, and sit/stand/kneel for physical assessments.
- Access to reliable transportation to travel up to 85% within licensed states for home visit assessments.
Preferred Qualifications:
- 1+ years of clinical experience in your highest level of education, clinical setting.
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
- Home Health care or home visit experience.
