Care Manager Outreach Services Specialist

Ulster County Community Action Committee seeks a dedicated Care Manager Outreach Services to support individuals in need within our community. As a key member of our team, you will play a vital role in connecting clients with essential services and resources to help them thrive.
This position offers a competitive pay of $20/hour. As a Care Manager Outreach Services, you will be responsible for conducting client assessments, developing individualized care plans, and coordinating services to meet clients' needs. You will collaborate with community partners, provide advocacy and support to clients, and maintain detailed documentation of all interactions.
Key Responsibilities:
- Conduct client assessments to identify needs and develop individualized care plans
- Coordinate services to meet clients' needs, including connecting them with community resources and advocating on their behalf
- Maintain detailed documentation of all interactions with clients and community partners
- Collaborate with community partners to ensure a comprehensive and collaborative approach to client care
