Healthcare Transition Specialist

We are seeking a skilled Healthcare Transition Specialist to join our team at Sea Mar Community Health Centers.
About the Position:
This full-time role will involve working closely with patients and healthcare providers to ensure seamless transitions between different levels of care. The successful candidate will have strong communication and interpersonal skills, as well as experience in case management and care coordination.
Key Responsibilities:
- Supporting patients and their families in navigating the healthcare system
- Collaborating with healthcare providers to identify and address gaps in care
- Developing and implementing personalized care plans to promote health outcomes
- Maintaining accurate records and reporting data on patient outcomes
- Staying up-to-date with relevant laws, regulations, and industry best practices
Requirements:
- Bachelor's degree in Human Services, Health Sciences, or related field
- Experience in social service case management or care coordination
- Ability to work independently and as part of a team
- Strong communication and problem-solving skills
