Job Overview

We are seeking a skilled Medical Case Manager (RN) to provide in-home nursing care for our clients in Austin, TX. As a member of our interdisciplinary team, you will assess patient needs, coordinate care, and ensure quality care and compliance with agency standards.

Compensation:

$46 - $50 per hour

ResponsibilitiesAssessments
  • Evaluate patients physical, psychosocial, and environmental needs in collaboration with the healthcare team.
Care Coordination
  • Deliver nursing care at the patient s residence, aligning with the plan of care and in collaboration with other healthcare professionals.
Infection Control and Compliance
  • Implement infection control and precautionary standards to prevent disease transmission during patient care.
  • Ensure all cases meet third-party payor requirements and agency quality standards.
QualificationsEducation and Experience
  • Bachelor s or Associate s degree in Nursing from an accredited RN program.
  • Current Texas RN license required.
  • Minimum of 2 years in a Clinical setting required (hospice experience preferred).
Skills and Abilities
  • Effective communication with patients, families, and healthcare teams.
  • Strong organizational and problem-solving abilities.
Transportation Requirements
  • Must have access to a reliable vehicle with active auto liability insurance for travel to patient locations.
