Job Summary

Centerstone is seeking a skilled and compassionate Foster Care Specialist to join our team. As a key member of our foster care services, you will play a vital role in ensuring the safety, permanency, and well-being of foster care clients.

Key Responsibilities
  • Conduct in-home visits and manage issues and concerns of foster children and families.
  • Link clients and families to community resources and provide linkage and referral to other services and supports to enhance family functioning.
  • Assist families with medication issues when necessary and help achieve all EPSDT and Dental requirements.
  • Complete all required face-to-face visits per DCS policy and perform documentation within 48 hours of contact.
  • Develop a plan of care within 30 days of admission and every 90 days thereafter, complete with goals, objectives, and interventions.
  • Use behavior management and crisis management skills when needed and coordinate case management services with school personnel.
  • Coordinate, transport, and supervise visitation as agreed upon in the CFTM and assist foster parents with transportation when needed.
  • Attend court as required and coordinate community service projects when necessary.
  • Complete required training and continuing education and participate in all staff supervision activities.
Requirements
  • Bachelor's degree required, preferably in Psychology, Social Work, Criminal Justice, or other behavioral health care field.
  • One to two years of experience with children, youth, and families.
  • Strong communication and writing skills, with the ability to work independently and as part of a team.
