Job Title: Community Health Nurse

Job Summary:

We are seeking a skilled Community Health Nurse to join our team at NYC Health + Hospitals. As a Community Health Nurse, you will be responsible for providing high-quality patient care in the comfort of patients' homes. You will work closely with patients, families, and healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including assessments, treatments, and education.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Monitor patient progress and adjust care plans as needed.
  • Document patient information and care plans in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Valid New York State license and current registration to practice as a Registered Professional Nurse.
  • Current certification in Basic Life Support (BLS) from the American Heart Association.
  • One year of experience as a registered nurse, preferably in home care or a related field.
  • Ability to travel regularly to patient assignments across New York City via public transportation and/or private vehicle.
  • Demonstrated commitment to continued professional development.

Preferred Qualifications:

  • Baccalaureate degree in nursing from an approved program.
  • Minimum two years of full-time nursing experience, preferably in home care, long-term care, or a hospital setting.
  • Demonstrated knowledge of Federal, State, and local regulations and JCAHO standards for home care.
