Aequor is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Skilled Nursing Facility (SNF) setting. As an LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our patients.

Responsibilities

  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Collaborate with other healthcare professionals to ensure seamless care transitions

Requirements

  • Current LPN licensure in Scotland, SD
  • 1+ year of experience in a Skilled Nursing Facility setting
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a team
