Job Summary:

As a Licensed Behavioral Health Case Manager with Kaiser Permanente, you will play a critical role in providing integrated and coordinated care to high-risk and high-utilizing populations in both inpatient and outpatient psychiatry and addiction medicine settings. Your expertise will be essential in managing all facets of care and utilization of these patients, ensuring they receive the best possible treatment and support.

Key Responsibilities:
  • Initial Intervention: Engage with high-risk, high-utilizing patient populations, acting as a central point of contact for referrals from multiple departments.
  • Planning of Services: Develop personalized plans based on assessments of short and long-term needs, tracking referrals and outcomes for ongoing quality management.
  • Coordination of Care: Facilitate collaboration between Primary Care, Behavioral Health Care Services, and Health Care Providers, providing referrals and follow-up services as needed.
  • Linkage with External Resources: Establish linkages and maintain working relationships with outside resources, providing linkage of members to community services.
  • Direct Patient Intervention: Provide individual and/or group therapy for chronic, high-utilizing patient populations, participating in Intensive Outpatient programs as a team member.
Requirements:
  • Minimum two (2) years recent experience in case management and working with community resources.
  • At least two (2) years experience in mental health settings and experience working in Inpatient and/or Outpatient treatment services.
  • Masters Degree in Social Work, Psychology, Sociology, or other related field related to behavioral health.
  • Licensed Professional Clinical Counselor (California) or Licensed Clinical Social Worker (California) or Licensed Marriage and Family Therapist (California).
Preferred Qualifications:
  • Bilingual Preferred (Spanish)
