About the Role

We are seeking a highly skilled Clinical Care Coordinator to join our team at Mass General Brigham. As a key member of our case management department, you will play a critical role in ensuring seamless transitions of care for patients.

Job Responsibilities
  • Facilitate discharge planning and coordinate post-acute care services for patients admitted to Medical, Surgical, Pediatric, or Obstetrical Services.
  • Develop and implement discharge plans for high-risk patients in collaboration with the patient/legal representative, healthcare team/provider, insurer, and community-based organizations.
  • Use nationally accepted criteria to determine medical necessity of admissions and appropriate level of care for hospitalized patients.
  • Review patients' health records and communicate with the healthcare team/provider to ensure continued appropriate level of care.
  • Complete timely and accurate insurance reviews as requested by third-party payers.
Requirements
  • Bachelor's degree in Nursing required; BSN preferred.
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts with a minimum of three years' clinical experience.
  • Evidence of continued education and professional development.
  • Previous experience in general acute care hospital settings, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery.
  • Prior experience in acute care hospital care coordination/discharge planning is essential.
  • Knowledge of special needs and behaviors of infant, child, adolescent, adult, and older adult patients is preferred.
