At WellLife Network, we are seeking a dedicated and compassionate Direct Support Professional to join our team. As a Direct Support Professional, you will play a vital role in providing care and support to individuals with intellectual and developmental disabilities, enabling them to live independently and productively.
Key Responsibilities:
- Provide all aspects of care, including personal care, meal preparation, and household management.
- Implement individualized care plans, ensuring each person's social responsibility, independence, dignity, and continued growth.
- Coordinate evacuation and ensure safety in emergencies, including fire drills.
- Administer medication and/or monitor self-administration of medication as a certified AMAP.
- Responsible for crisis intervention and incident reporting procedures.
- Monitor and record progress on daily checklists, behavior contracts, and quarterly summaries.
- Communicate relevant information to other staff and supervisors, verbally and in writing.
- Meet with individuals and management teams to review goal progress, identify needs, and update program plans.
- Assist individuals in researching and obtaining community resources and planning recreational activities.
- Provide training in ADLs and other areas, perform tasks for individuals requiring total support, and act as a role model.
- Maintain confidentiality and follow up on aftercare recommendations.
- Work schedules and locations are subject to change based on individual and program needs.
Qualifications:
High School diploma or equivalency diploma required. A valid New York State Driver's License with 2 years of driving experience is required. Experience working with adults with intellectual/developmental or other behavioral health disabilities is preferred.
