At St. Croix Hospice, we are seeking a compassionate and skilled Hospice Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.

Key Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care by utilizing the nursing process and collaborating with other healthcare professionals.
  • Write and initiate care plans, and participate in revising them as necessary.
  • Initiate and coordinate patient care, including administering medications and treatments as prescribed by physicians.
  • Counsel patients and families on meeting their nursing and related needs.
  • Provide health care instructions to patients as appropriate.

Requirements:
  • Graduate of an accredited school of nursing.
  • One year of hospice experience, preferred.
  • Associates degree required, bachelor's degree preferred.

Working Conditions:
  • Able to lift supplies and equipment.
  • Visual acuity and hearing to perform required nursing skills.
