Job Summary:
We are looking for a skilled Physical Therapist to work with patients in their homes, delivering personalized care that addresses their unique needs and goals. As a member of our team, you will be responsible for conducting thorough assessments, developing effective treatment plans, and collaborating with healthcare professionals to ensure seamless care delivery.

Duties and Responsibilities:
• Conduct comprehensive evaluations to determine patient needs and develop individualized treatment plans
• Provide one-on-one physical therapy sessions, focusing on pain management, mobility, and functional improvement
• Collaborate with healthcare teams to share progress updates and adjust treatment plans as needed
• Educate patients and their families on proper techniques, exercises, and lifestyle modifications for optimal outcomes
• Maintain accurate records and documentation of patient progress and treatment plans

Requirements and Qualifications:
• Current licensure as a Physical Therapist in the state of practice
• CPR certification and access to a reliable vehicle
• Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
• Ability to lift 25 pounds and navigate various environments comfortably

Salary and Benefits:
