Salary

The annual salary range for this position is $50,000.00-$55,000.00, with opportunities for growth and advancement within the company.

Job Description

As a Homecare Nurse, you will be responsible for providing comprehensive care to patients in their homes, including:

  • Performing initial nursing assessments on new admissions
  • Completing, coordinating, and revising the total plan of care and maintaining continuity of patient care by collaborating with appropriate staff
  • Regularly communicating patient progress to the clinical manager, physician, and care team
  • Developing, preparing, and maintaining individualized patient care progress records with accuracy, timeliness, and according to care center policies
Required Skills and Qualifications

To be successful in this role, you must possess:

  • A current RN license, specific to the state(s) you are assigned to work
  • One+ year of nursing experience required
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
