About the Role

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team in Spokane Valley, WA. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with an interdisciplinary team to deliver whole-person care.

Key Responsibilities
  • Provide direct patient care, including assessments, medication management, and wound care.
  • Develop and implement individualized care plans, taking into account patient goals and preferences.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and satisfaction.
Requirements
  • Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN).
  • Current Washington State Registered Nurse License.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • National Provider Identifier (NPI) number within 30 days of hire.
Preferred Qualifications
  • Bachelor's Degree in Nursing (BSN).
  • 3-5 years of experience in acute care or community behavioral health settings.
  • Community behavioral health and geriatric experience.
