About the Role:
As a Primary Care Physician at Conviva, you will be responsible for providing high-quality, patient-centered care to a diverse population of seniors. Your expertise and leadership skills will enable you to manage a panel of patients with complex medical needs, while collaborating closely with other healthcare professionals to ensure seamless care coordination.

Key Responsibilities:
- Provide primary care services to patients in the Corpus Christi market
- Lead a multidisciplinary care team to deliver value-based, senior-centric healthcare
- Develop and implement effective care plans to address the unique needs of each patient
- Collaborate with specialists and other healthcare professionals to ensure comprehensive care
- Stay up-to-date with the latest medical research and best practices to provide evidence-based care

Requirements:
- Current and unrestricted medical license in the state of Texas or willingness to obtain
- Board certification or board eligibility in Family Medicine or Internal Medicine (Geriatric Medicine preferred)
- Excellent communication and interpersonal skills
- Ability to work effectively in a team-based environment
- Strong analytical and problem-solving skills
