Job OverviewWe're seeking a highly motivated and experienced LPN to join our team at a skilled nursing facility in Newport, VT. The ideal candidate will have a strong foundation in nursing principles, excellent communication skills, and a passion for delivering high-quality patient care.
Responsibilities- Delivering Direct Patient Care: You'll work under the guidance of RNs or physicians to administer medications, perform wound care, and monitor vital signs.
- Comprehensive Patient Assessments: Conduct thorough physical exams, medical histories, and medication reviews to craft personalized care plans that meet each patient's unique needs.
- Medication Administration: Administer prescribed medications and treatments with accuracy and attention to detail, ensuring compliance with established protocols.
- Wound Management: Evaluate, clean, dress, and manage wounds, incisions, and injuries while monitoring for infection or complications.
- Vital Sign Monitoring: Continuously monitor and document patients' vital signs, including blood pressure, pulse, temperature, and respiration rates.
- Patient Education: Educate patients and their families on medical conditions, prescribed medications, and self-care practices to promote overall well-being.
- A valid and active Licensed Practical Nurse State license
- BLS certification
