Job Summary

We are seeking a skilled Palliative Care Social Worker MSW PRN to join our team As a Palliative Care Social Worker MSW PRN, you will play a vital role in providing direct casework and counseling services to patients and their families, working closely with healthcare professionals to ensure comprehensive care.


About the Role

The Medical Social Worker provides direct casework and counseling services to the patient/family unit. This includes obtaining evaluations of psychosocial problems, defining social service goals for alleviating these issues, and communicating effectively regarding end-of-life conversations and discussing goals of care with the patient/family.


Essential Job Functions/Responsibilities

  • Perform psychosocial assessments of patients, families/caregivers in a timely manner, identifying their needs and developing appropriate interventions.
  • Educate and assist patients/families with advance directive documents, ensuring they understand their options and rights.
  • Counsel patients and families/caregivers with end-of-life concerns, providing emotional support and guidance on coping skills.
  • Provide assistance and follow-up with applications for Medicaid, Provider Services, and other community-based services, helping patients access the resources they need.
  • Request spiritual support for patients and families as needed, recognizing the importance of holistic care.
  • Attend interdisciplinary team meetings, collaborating with healthcare professionals to create, review, and revise patient care plans.
  • Communicate effectively with hospitals, doctors, home health agencies, State Adult Protective Services, and other providers, ensuring seamless coordination of care.
  • Develop and maintain relationships with community providers, serving as a liaison between patients/families and healthcare services.
  • Stay up-to-date on Medicare and Medicaid rules and regulations, explaining the benefits and eligibility criteria to patients/families.
  • Maintain accurate and relevant clinical notes, physical examination notes, assessment visit notes, collaboration notes, and other documentation in the medical record.


Position Qualifications

  • Master's degree in Social Work from a Council of Social Work Education (CSWE) accredited school of social work or a school in candidacy status by the CSWE.
  • Licensed Clinical Social Worker (LCSW) with the state of Oregon.
  • Two years of relevant Social Work experience, including Clinical Social Worker Associate (CSWA) experience.
  • Advanced Certified Hospice and Palliative (ACHP) Social Worker certification is preferred.
  • Experience working with individuals with serious illnesses and a strong understanding of palliative care principles.
  • Excellent observation, verbal and written communication skills, problem-solving skills, mathematical skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and supply lifting and equipment handling.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Will require passing background checks and drug tests.
