About the Role

We are seeking a highly skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our team in providing comprehensive medical care to patients in skilled nursing and long-term care facilities. Responsibilities

  • Conduct thorough initial assessments, develop personalized care plans, and perform routine follow-up evaluations to ensure optimal patient outcomes.
  • Manage acute and chronic medical conditions, including diabetes, hypertension, respiratory diseases, and neurological disorders, and prescribe appropriate medications and treatments as needed.
  • Collaborate closely with attending physicians, nursing staff, physical therapists, and other healthcare professionals to coordinate comprehensive patient care plans.
  • Maintain accurate and up-to-date medical records, ensuring compliance with facility policies, state regulations, and billing requirements.
  • Educate patients and their families on disease prevention, treatment options, medication adherence, and lifestyle modifications to promote overall well-being.

Requirements

  • Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the state of NY.
  • Certification as an NP or PA from an accredited program.
  • Previous experience in geriatrics, rehabilitation, or long-term care is preferred, but not required.
  • Strong clinical assessment skills, ability to make autonomous clinical decisions, and excellent communication skills are essential.
