Home Health RN Case Manager

We are seeking a skilled Registered Nurse to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing high-quality patient care and coordinating with healthcare professionals to ensure seamless transitions.
ResponsibilitiesAs a Registered Nurse Case Manager, you will:
- Perform home visits and coordinate patient care with healthcare professionals
- Oversee the frequency of visits for each patient episode
- Consult with physicians and office staff to ensure accurate and timely patient care
- Collect and analyze OASIS data to inform care decisions
- Ensure accurate medication profiles and timely lab value reporting
- Attend weekly case conferences and monthly case manager meetings
To be successful in this role, you must:
- Be a currently licensed Registered Nurse in the state of employment
- Have a minimum of one year of clinical experience
- Demonstrate knowledge and skill in current nursing practice
- Possess a valid state driver's license and automobile liability insurance
