Physical Therapist Job Description

Elderwood at Burlington is seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in ensuring the health and well-being of our residents by providing comprehensive and quality therapy services.

Responsibilities:
  • Administer skilled physical therapy treatments in accordance with physician's orders.
  • Maintain written evaluations, progress notes, and discharge summaries for individuals in physical therapy program.
  • Confer with other healthcare staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
  • Provide input for meetings and attend during absences of the Director Physical Therapy.
  • Maintain statistical records of individuals' treatments.
  • Assist with in-service training programs upon request.
  • Record and update individual's plan of care in the Inter-disciplinary Kardex.
  • Submit requisition for supplies needed in the treatment to Director Physical Therapy.
  • Assume all duties of the Director Physical Therapy during absences, including supervision of Physical Therapy Aides and responsibility for all necessary documentation.
Qualifications:
  • Graduation from an approved school of physical therapy required.
  • State license to practice physical therapy required.
  • Two years' experience in physical therapy preferred.
  • Ability to direct others necessary.

This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
