Job Summary:
We are seeking a skilled Per Diem Registered Nurse to join our team at Providence Elderplace in Beaverton, OR. As a key member of our interdisciplinary team, you will provide care management, care coordination, and skilled care services to vulnerable elder patients with complex medical and psychosocial issues.
About the Role:
This per diem position is ideal for experienced nurses who want to make a meaningful impact in their community while enjoying a flexible schedule. You will work closely with our patient-centered team to ensure a systematic approach to health services, improve patient outcomes, and enhance quality of life.
Responsibilities:
- Provide nursing care and support to patients in their homes or in congregate living settings
- Care plan development, implementation, and ongoing assessment
- Collaboration with healthcare providers, family members, and caregivers to ensure seamless care coordination
- Identification of needs and coordination of access to appropriate care and services
- Maintenance of accurate records and documentation
Requirements:
To be successful in this role, you must have an Oregon Registered Nurse License and National Provider BLS certification within 30 days of hire. Relevant certifications applicable to the position are also required upon hire. A Bachelor's Degree in Nursing is preferred, along with 1 year of home health RN experience.
Compensation and Benefits:
We offer a competitive salary range of $40.20-$63.64 per hour, depending on experience. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
