Job Summary:

This position requires a Registered Nurse with knowledge of managed care and population-based disease management and prevention concepts/strategies. The successful candidate will work collaboratively with physicians, staff, and other healthcare professionals to provide care coordination across the healthcare continuum for a high-risk population within the clinic setting.

Key Responsibilities:

  • Collaborate with healthcare teams to develop and implement care plans for high-risk patients
  • Utilize data to identify trends and areas for improvement in patient care
  • Work closely with the Practice Transformation Quality team to ensure high-quality standards of care are maintained
  • Provide exceptional interpersonal customer service to patients, families, and colleagues
  • Participate in ongoing education and training to stay current on best practices in population health and disease management

Requirements:

  • Minimum three (3) years' experience in a hospital, home care, or managed care setting
  • Strong knowledge of managed care and population-based disease management and prevention concepts/strategies
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a healthcare team
