Healthcare Coordinator Job Summary

We are seeking a skilled LVN to join our team as a Healthcare Coordinator. In this role, you will work closely with patients, healthcare providers, and multidisciplinary teams to ensure seamless transitions of care and optimal health outcomes.

Key Responsibilities:

  • Identify patients at high risk and develop strategies to support their care
  • Perform assessments, initiate medication reconciliation, and provide patient education
  • Support transitions of care from inpatient to outpatient settings
  • Collaborate with providers to establish and update plans of care
  • Document patient information accurately and timely in electronic medical records

Requirements:

  • Licensed Practical/Vocational Nurse with active and unrestricted license
  • Current BLS certification
  • 2+ years of experience in a physician's office, clinical, or hospital setting
  • Knowledge of chronic diseases and electronic medical records

Preferred Qualifications:

  • IV Certification
  • Experience with patient education and motivational interviewing skills
  • Fluent written and verbal skills in English and Spanish

Physical & Mental Requirements:

  • Ability to stand for extended periods of time
