Job Summary:

We are seeking an LPN MDS Coordinator to participate in the Resident Assessment Process through the completion of pertinent items on the Minimum Data Set (MDS) and accompanying Care Area Assessments (CAAs). This role involves ensuring appropriate patient care and nursing center reimbursement for the care provided.

Key Responsibilities:
  • Direct the interdisciplinary Resident Assessment Instrument (RAI) process while following guidelines for completion, computer encoding, and electronic State transmission of the MDS.
  • Participate in developing individualized resident Care Plans, identifying resident needs and projected outcomes as required by Federal and State regulations.
  • Train new members of the interdisciplinary team on clinical content and electronic completion of applicable MDS items.

Requirements:
  • Must be an LPN in good standing and currently licensed by the state.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
  • Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations, and guidelines governing long-term care.
  • Must have computer skills to include Windows applications.
  • AccuMed experience is preferred.
  • Previous experience as an MDS Coordinator is preferred.
  • Prior experience in completing MDSs and Care Plans is required.
  • Demonstrated proficiency in RAI/PPS processes is required.
  • Must maintain current BLS/CPR certificate (excluding ALFs/ILFs).

What We Offer:
A competitive salary range of $36.00 - $40.00 per hour, depending on experience and qualifications.
