Job Summary:

We are seeking a skilled MDS Nurse to join our team at Autumn Care of Myrtle Grove. As a key member of our healthcare staff, you will be responsible for completing the Resident Assessment Instrument (RAI) process and providing high-quality care to our residents.

Responsibilities:
  • Complete the RAI process in accordance with state and federal regulations
  • Develop and implement individualized care plans for residents
  • Collaborate with interdisciplinary teams to ensure seamless care delivery
  • Monitor and document resident progress and outcomes
Requirements:
  • Active, unencumbered Registered Nurse license in North Carolina
  • Minimum one year experience in long-term care nursing
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
