Responsibilities:

  1. You will participate in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
  2. You will coordinate patient care including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients.
  3. You will perform ongoing analysis of patient data with each patient visit and document in the patient medical record.
  4. You will adjust or modify the treatment plan as indicated and notify supervisor as needed.
  5. You will provide initial and ongoing education to patient and family.
  6. You will administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness.
  7. You will initiate or assist with emergency response measures.
  8. You will ensure correct laboratory collection, processing, and shipping procedures are performed.
  9. You will collaborate with the Interdisciplinary Team on the patient care plan.

Requirements:

  • You must have a graduate degree from an accredited School of Nursing.
  • You must have current appropriate state licensure.
  • You must have current or successful completion of CPR BLS Certification.
  • You must meet the practice requirements in the state in which he or she is employed.

Salary and Benefits:

The estimated annual salary for this position is $95,000-$120,000 based on location and experience. Fresenius Medical Care offers a comprehensive benefits package, including health insurance, 401(k) matching, and paid time off.
